Cataract

Cataract surgery is one of the most frequently performed surgical procedures worldwide and, therefore, significantly impacts public health. With the aging population, the quantity of performed operations per year will only grow. Although modern technology has evolved enough to make the surgery safe, but there are obstacles in cataract surgery that still need improvement in order to ensure refractive outcomes, especially when it comes to implanting advanced IOLs.

IOL Tilt and Decentration

Advanced IOLs provide excellent visual quality after the surgery, and they are able to provide spectacle independence in many cases, which brings patients’ lives to a new level. Nevertheless, many surgeons remain reluctant to offer them because of the demanding selection process and difficulty reaching targeted refraction. The rate of IOL tilt and decentration is often underestimated since there is no standardized approach to evaluate the IOL position accurately during or after implantation. Therefore, this examination has not been included in the postoperative routine until now

Lens Reviewer

We developed an innovative device that closes the gap in IOL tilt and decentration measurements and facilitates fast, non-contact, accurate assessment of the IOL position inside the capsular bag relative to visual axis. The Lens Reviewer makes the postoperative measurement of IOL tilt and decentration more intuitive and straightforward than ever before. Broad adoption of our device can eliminate unnecessary treatment and minimize the need for post-cataract refractive surgeries, arming surgeons with the measurements to correct IOL position.

The decentration above 0.3 mm negatively affected visual quality due to increased tHOAs, spherical aberrations, coma and secondary astigmatism aberrations, the influence become larger with increasing pupil diameter.

Diao, C., Lan, Q., Liao, J. et al. Influence of decentration of plate-haptic toric intraocular lens on postoperative visual quality, 2023

Tilt accounts for more than 10° of the error in toric IOL power calculation and this value increases to almost 20%, if combined with angle kappa

Hirnschall, N., Findl, O. Lens and IOL Tilt, 2024

The main source of error in toric IOL power calculation is the preoperative corneal measurement followed by IOL misalignment and tilt.

Larger extent of tilt and decentration has a negative impact on the optical performance and subsequently, the patients' satisfaction.
